What documents are needed to apply for a passport?

Following documents are required to apply for US passport:
  • Application Form DS-11.
  • Proof of US citizenship using US birth certificate or Naturalization Certificate.
  • Proof of identity such as passport, naturalization certificate, certificate of citizenship, current valid driver's license.
  • 2 passport photos.
  • Application fee.

Thereof, do I need a birth certificate to get a passport?

If you were born in the United States and do not have a birth certificate to submit with your passport application, you may submit the Form DS-10, or a Birth Affidavit as evidence of your U.S. citizenship. The birth affidavit: Must be notarized. Must be submitted in person with Form DS-11.

Can you use a copy of a birth certificate to get a passport?

Your citizenship evidence will be mailed back separately from your new passport. It states on my U.S. birth certificate that it is illegal to make a copy. If you choose not to submit a photocopy, you may submit a second certified copy of your birth certificate.
